JOB DESCRIPTION

The Support Coordinator is responsible for connecting people with a National Disability Insurance Scheme (NDIS) Support Plan to access a range of supports which align with the support items in their NDIS plan and that suit their individual needs.As a Support Coordinator you will provide varying levels of coordination and transition support for young people and adults, giving consideration to individuals circumstances and level of complexity.The Supports Coordinator is a valued member of our team and contributes to the development of service practice through regular staff meetings, planning processes and policy reviews. All service delivery must be in line with the Policies and Procedures of Total Support Services and the requirements of State and National Disability Services Acts.PURPOSETo work in partnership with clients on improving and evaluating the delivery of services;Attract new clients to the service by promoting services and accepting requests for support coordinator;To provide direction and supervision to staff to ensure our clinical practise meets the needs of our clients;Develop and maintain relationships with all stakeholders;Deliver quality services to clients in line with policies and procedures.KEY RESPONSIBILITIESClinicalTo be a productive member of the Total Support Services and provide a range of therapeutic services to clients;Provide crisis intervention services, conduct psycho social assessments and make recommendations for care and intervention;Develop and review support plans to ensure they meet the needs of the clients;Provide supervision and direction to support workers to ensure they are working in line with the support plans;Maintain accurate records of involvement and contribute to Assessment reports;Attendance at care team meetings and provide written reports as required.Communication and Team WorkEstablish therapeutic relationships with clients and other stakeholders;Establish working relationships with medical, nursing and allied health staff, community clinicians and other service providers;Provide feedback and training to staff on the outcome of assessment and recommendations;Actively participate in client meetings to ensure effective communication and dissemination of information;Foster a culture of openness, respect, accountability and professionalism in the workplace;Represent the Total Support Services inter agency meetings and other meetings as negotiated.Client ManagementMaintain client documentation including:Support plans completed annually or more frequently if required;Maintain file note for each contact with clients;Review client file notes, ensuring they are factual and appropriate;Provide advocacy for clients and negotiate funding for clients where required;Co-ordinate supports to ensure services are provided in line with funding and individual support plans.MANDATORY SELECTION CRITERIAMSC1: A degree in Human Services or equivalentMSC2: Current driver’s licenceMSC3: DCSI Screening ClearanceKEY SELECTION CRITERIAKSC1: Demonstrated skills in counselling both children and adults.KSC2: Demonstrated knowledge of evidence based practiceKSC3: Extensive experience in Social Work practice & demonstrated ability and understanding case managementKSC4: High quality written and oral communication skills & strong computer skillsKSC5: High level of effective communication with stakeholdersDESIRABLE SELECTION CRITERIADSC1: Good working knowledge of child protectionDSC2: Good working knowledge of NDISContact DetailsTitle*First Name*Last Name*Phone*Email*Street Address*City*State / Territory*Please SelectNSWVICQLDWASANTTASACTOtherPostal Code*Application Section1.
