Your Community Health is seeking an enthusiastic and experienced Occupational Therapist (Grade 2) to join the Occupational Therapy Team on an ongoing, part-time (0.7 FTE) basis. Experienced Grade 1 Occupational Therapists are encouraged to apply.

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About The Role

Reporting to the Occupational Therapy Coordinator, the Occupational Therapist will provide high-quality home and community-based Occupational Therapy services to clients of Your Community Health.

Your Passion And Commitment Will See You

Provide evidence-based client assessment and intervention to maximise functional independence, safety and quality of life for diverse client groups Select, prescribe and order assistive technology (including home modifications) as appropriate and seek funding through appropriate funding schemes Liaise and work cooperatively with internal and external service providers Work autonomously to manage a varied caseload and meet deadlines and waiting times

To Succeed In This Role You Will

Have recognised qualifications as an Occupational Therapist (OT) and be eligible for membership of the Occupational Therapy Association of Australia Be a registered Occupational Therapist with AHPRA Be registered as a provider with the State-Wide Equipment Program (SWEP) Have well-developed verbal and written communication skills, including the ability to prepare concise reports Have experience as an Occupational Therapist, providing assessment and intervention for diverse client groups.
